For our patient’s convenience, Easton Family Practice has made all of our
new patient forms accessible on the Internet. A new patient is now able to
access all the necessary forms, and fill them out before stepping into the
office. This would save time sitting in the doctor’s office going over form
after form. All of the forms listed below are necessary for the medical
